Description
Provides comprehensive care for children with SCD under a "one-stop shop" medical home. The 360 degree care demonstrates the principles of DC-GAPS which include: building bridges to overcome gaps and restoring lives and through mentoring and support to children with sickle cell disease to help them achieve their full potential.
