CrossOver specializes in the treatment of chronic diseases, proving comprehensive treatment plans for patients suffering from diabetes, hypertension, chronic obstructive pulmonary disease, cardiovascular disease, and other chronic conditions through the Chronic Disease Management Program. Through a partnership with the VCU School of Pharmacy, also offer intensive diabetic management through the ambulatory care clinical pharmacist (PharmD) program.
