Virginia resident, under the age of 21, have special health care needs as defined by program guidelines.

Application process

Referral can be made by anyone, parents, students, health care providers and others, by calling the central number to make referrals.

Description

Case managers work closely with children and their families to provide targeted, comprehensive and culturally appropriate care. The case managers coordinate all services offered by the various providers to children in need, and actively seek input from families and medical practitioners to achieve the best possible results. Each Care Connection for Children program offers: coordination, information, education, social support and referrals.
