Services for adults with intellectual disabilities. Assesses consumer strengths and needs, links consumers with community services and supports, monitors consumer's response to treatment and the quality of care provided, facilitates Person Centered Planning to fulfill a consumer choice of employment, living options and socialization, and ensures crisis stabilization services if needed.
