CHCG provide individualized training and support to individuals in-home or in a comfortable non-restrictive environment. Assist in developing real world problem solving techniques to encourage independence, personal and community stability. Services will be provided in three month increments with a maximum of ten hours weekly. Mental Health Skill building services include: Independent living skills Hands on activities to manage daily living Medication management Financial management Monitor Personal Hygiene Social skills Organization Maximizing community resource
