Eligibility

Active Duty and Retired military and their ID card family members; and Reservists and National Guard (while on active duty, Title 10 for more than 30 days) and their ID card family members.

Hours

Closed the first Wednesday of the month 8AM-12noon.

Application process

Priority is given to appointments. Walk-ins are seen on a case-by-case basis. We highly encourage calling first to ensure that staff is available to serve you.
