Program is offered for youth and children who are at risk of being removed from their homes due to neglect or abuse, delinquency, behavioral/emotional/mental health problems or substance abuse. Program is also offered to youth who are transitioning back home from foster care, correctional facilities, or residential treatment. Services include: several contacts per week; individual and family sessions in the home; collateral contacts with schools and other service providers; individualized treatment plans; aftercare planning; and comprehensive needs assessment.
