Eligibility
Persons 60 years of age and older who are frail, have disabilities, or who live in isolated areas, with particular attention to those with greatest economic and social need, low-income minority individuals, and individuals with limited English proficiency.

Fees
Persons having an income at poverty level there is no charge. Persons having income over the poverty level pay on a sliding fee scale after deductions are given for medical expenses.
