The DARS Community Rehabilitation Case Management Services (CRCMS) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. Carilion Clinic Adolescent and Student Health Service's mission is to help young adults stay healthy now and in the future. We serve youth ages 10-26 and offer a wide range of healthcare services including wellness visits, school, sports, and camp physicals, acute illness and minor injury treatment, immunizations, diagnosis and treatment of sexually transmitted diseases, contraception and family planning, mental health counseling, and health educaton services.

Through the group practice concept, even though the patients personal physician may not be available, his partners in the group will have access to his records, ensuring optimum direction and provision of care. Clients who wish an individual physician referral are referred to the Norfolk Academy of Medicine or to the Medical Society for the city in which they live. Also provides nutrition services and home health consultation as needed.
