Healthy Families Fairfax is for expectant parents or families with a newborn who wants help to be the best parent possible. A family support worker will visit you in your home on a regular basis before your child is born and continue to work with you until your child is ready for preschool or kindergarten.

The Resource Mother is a community health worker who develops a supportive mentoring relationship with the teen and her family. From the prenatal period through the infant’s first birthday, the resource mother provides health education; discusses ways to prevent infant injury; models daily living skills; encourages constructive decision making and life-planning; connects the teen to community resources; and provides guidance to assist the teen in making a successful transition to parenthood. The Resource Mother has weekly contact with the teen and her family.
This program uses a two-generation or three-generation approach to serve target populations. Educational and support services are also available to other family members such as the partner, the teen's parents, and the infant.

Families are typically referred prenatally or at the time of the baby's delivery. Families will receive an initial visit, during which their strengths and needs are identified. A determination is then made about whether Healthy Families or other services in the community are right for them. Families are voluntarily connected to community resources they prefer.
Families who do best in the program are willing to receive at least one home visit per week for the first six months after the baby's birth. After that the frequency is determined by their needs and progress. Families have the choice to receive services until the child enters preschool or until the child's fifth birthday. Families must enroll prenatally or within three months of birth. There are no income restrictions.
