The 111-bed unit provides inpatient rehabilitation and recovery care for those unable to receive rehab at home, who may or may not have a condition severe enough to enter long-term care. The DARS Community Rehabilitation Case Management Services (CRCMS) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. A 60 bed skilled nursing facility with a focus on delivering premiere ventilator care and rehabilitative services for residents with moderate to chronic medical conditions. Offers short term, post acute inpatient rehabilitation.

This program is ideal for short-term, post-acute rehabilitation after a hospital stay. Comprehensive rehabilitative services are administered by a full team of licensed physical, occupational and speech therapists. Provide patient-specific programs that are designed to achieve and maintain the patient independence.

Provides community-based case management for individuals affected by brain injury. Case management includes identifying needs and developing a plan of action to meet those needs. Help individuals and their caregivers find their way through the rehabilitation process which can last for years following an injury. After determining what an individual needs and would like to accomplish, we identify and coordinate services to help them reach their goals. Monitor progress toward those goals and build partnerships and teams to create a lasting network for continued support. Primary function is to help individuals affected by brain injury return to their lives - to their homes, careers, school and communities - to the greatest extent possible.
