Needs assessment, case management, and referral to a network of providers to help youth exhibiting negative behaviors. TOP (Teamwork ~ Options ~ Pathways to Success) is an interest and needs based prevention program designed to connect youth and their families with services that can refocus youth in the right direction by building on their strengths and providing them with a positive support system.

The DARS Community Rehabilitation Case Management Services (CRCMS) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. A Medicaid approved program provided by the Alexandria Health Department to foster safe and healthy pregnancies and healthy babies by coordinating services and providing education.

Mental health case management works to ensure that service systems and community supports are responsive to the changing needs of adults with serious mental illness. This service seeks to assist individuals in gaining access to needed medical, psychiatric, housing, financial, social, educational, vocational, and other supports essential to meeting basic needs. The individual is a partner in assessing their own needs, obtaining services and other supports, monitoring those services, and in preventing and managing crises. Case management utilizes a person-centered approach with a focus on promoting health and recovery.

Provides case management, assessment and direct services to Virginia's citizens who have disabilities or who are age 60 plus. Specific services include: Home-based services (companion, chore, and homemaker services); Adult foster care; Adult day care; Case management; Nutrition; Assessment for assisted living facility applicants and residents; Participation on nursing home pre admission screening teams.

The goal of case management is to optimize client functioning by providing services to individuals with multiple and complex needs. Case management helps individuals make changes to improve their life situation.

Case management team works with survivors to develop a person-centered plan for success and work with individuals to help locate and access supports in the community.

Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to assisting in placement such as an adult home, nursing homes, etc.; companion services designed assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization. Region Ten's Adult Developmental Services Center offers a wide range of person-centered services designed to support individuals with developmental disabilities in living a safe, healthy, and productive life as independently as possible. These services include case management, group home residential, sponsored residential, in-home support services, independent living supports, day support services, and community engagement.

Good Shepherdof Northern Virginia provide appropriate resources to prevent or address homelessness and teach life skills required to help individuals and families return to stability for long term success. The Six Steps to Self-Sufficiency program is an intensive case management approach to goal-setting for long term achievement of stability for households.

Case managers assist children and their families in finding medical, psychiatric, social, educational, and other services which are needed for the child to thrive in the community. This would include help with referrals to other services, transportation to appointments, and assistance in working with school staff, physicians, and other agencies. Case managers work with children with serious emotional disturbances. They help families access outpatient treatment, intensive in-home treatment, and ongoing psychiatric treatment and medication. Case managers also assist children with mental retardation to gain access to the Mental Retardation Waiver, which can provide personal assistance, respite, assistive technology and other services. Case managers also work with a child's IEP team in the schools.

VPAS' Case Management service offers an assessment of needs, development of an individualized plan of care, referral and linkage to community services, and monitoring and satisfaction of services implemented. A personal home visit is conducted to determine the level of service required.

The Pediatric Case Management Program services children, teens, and young adults up to age 25. Services are tailored to meet the individual needs of each client and their family or support system. Services support families, schools, and community agencies in providing the best opportunities for appropriate development for a child, teen, or young adult student with a brain injury.

Provides comprehensive substance use services such as medication-assisted treatment, case management, and individual, family, and group counseling.

Provides access to information/education, resources, linkage to care, and services in the prevention and care of HIV/AIDS and Hepatitis C. Person-centered Aging Services Coordination is an interactive decision support process where people are supported in making long-term support choices that reflect their unique preferences, strengths, needs, values, and circumstances. Aging Services Coordination is provided by individuals trained and practicing according to Virginia's Statewide Standards.

DePaul Community Resources is an independent, non-profit human services organization which serves the communities of Central and Southwest Virginia through our Treatment Foster Care, Developmental Disabilities, Clinical Services, and Adoption programs.

Provides case management/care coordination for both mental health and substance abuse. Services are designed to link individuals with resources in order to promote community integration and independence while promoting self-determination and recovery.

Partners with school systems and other agencies to help students with disabilities in transition from middle school to high school and post graduate opportunities. Helps people transition from nursing facilities and other institutions back into the community.

Offers Warm Hearth at Home, a senior care management services program. Assists older adults to live as independently and as safely as possible through the coordination of needed services. Provides the additional support necessary to assist clients to maintain their current living arrangement or can help to prepare clients to navigate the transition to a higher level of care when needed.
