Case management team works with survivors to develop a person-centered plan for success and work with individuals to help locate and access supports in the community.
Provides programs to children ages 6-18 and their families who live in Newport News. Services include programs in -education: parenting classes and workshops; GED classes; workforce development; teen responsibility groups; computer training; after school program with academic assistance and enrichment; SUPER Saturday enrichment program; and scholarship awards -health care services (in collaboration with Peninsula Institute for Community Health, Peninsula Health District and Hampton University Nursing Department): health screenings and referrals; health education; dental screenings and referrals -creative services (in collaboration with Newport News City of Parks and Recreation): cultural activities; leisure activities; instructional recreation and fitness; spring break and summer camps -case management services: individual/group supportive counseling; individual/family assessments; program development; home and school visits; referral services; community outreach

Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to : assisting in placement such as in an adult home, nursing homes, etc.; companion services designed to assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization.

Provides care management services to elderly persons and their adult children. Services include consultations, psychotherapy and counseling with adult children and older people, visits and assessments in the home, nursing home, or hospital. Locates resources in the family's or the relative's community. Maintains a national network of private geriatric care managers. Offers initial one-half hour introductory consult at no charge.

Adult Services include the provision of companion services to the elderly and screening to determine the need for possible nursing home or adult home placements.
VPAS' Case Management service offers an assessment of needs, development of an individualized plan of care, referral and linkage to community services, and monitoring and satisfaction of services implemented. A personal home visit is conducted to determine the level of service required.
The Exceptional Family Member Program (EFMP) provides assistance for active duty Marine Corps family members that require special medical and/or educational services based upon a diagnosed mild to severe physical, psychological or educational need.

Entry point for Ryan White Part B Services. Case Management and Patient Navigation services for persons living with HIV or HCV. Medical clinic, emergency housing/utility assistance, transportation vouchers, and food pantry for those living with HIV ONLY. CHARLII (Comprehensive HIV/AIDS Resources and Linkages for Individuals experiencing Incarceration) coordinates for persons recently released from incarceration who are HIV positive.

Provides a program where case workers set up plans for continued medical care for patients on leaving the hospital. Provides crisis intervention, information and referral.
Assess the needs of the client and recommend services which will help enable the client to live as independently as possible. The Case Management Department will then coordinate the needs of the client with all available LCAAA and Community Resources. At the time of the initial assessment, a Plan of Care will be formulated, and all clients will be reassessed annually or as their conditions/needs change. Case Management clients will be reassessed bi-annually.
Community Support Programs-Case Management is composed of programs designed to provide community based Behavioral Health services to seriously mentally ill adults and those with substance use disorders Program activities support consumer recovery and community integration by providing a continuum of services designed to promote maximum independence in the community. Services are tailored to respond to the unique needs of individuals with serious mental illness.

The Care Coordination Program assists older adults (and their families) by helping to coordinate and organize the care they are receiving from various agencies and organizations. Coordinators provide information and referrals; collaborate with other service professionals; and develop person-centered service plans that focus on participants' needs, strengths, and preferences. They also follow up with individuals each month to ensure those plans are working well and adjustment them if any needs have changed.

Work with individuals and caregivers to develop person-centered plans to meet the individual's needs so she/he can age in place in their community of choice. Some of the supports and services offered may include home-delivered meals, transportation to medical appointments, referral for in-home personal or homemaker care, wellness classes, or other community-based services.
Provides case management, assessment and direct services to elderly citizens (60+) and to adults with disabilities. Services include assessment for assisted living facility applicants and residents, participation on nursing home pre-admission screening teams, home-based services (homemaker services), referrals for adult day care and the Senior Center, investigations for Adult Protective Services and Case Management. Also provides services to adults so that they can remain in the least restrictive environment.
Program offers people with developmental disabilities a family based solution to their needs. Family Care homes provide a person-centered, family-based home in which an individual with developmental disabilities can become an active member of a family. Respite support offers temporary, short-term, home-based support for an individual. Respite offers care givers an opportunity for vacation and support during emergencies.
