The Case Management Department will then coordinate the needs of the client with all available LCAAA and Community Resources. At the time of the initial assessment, a Plan of Care will be formulated, and all clients will be reassessed annually or as their conditions/needs change. Case Management clients will be reassessed bi-annually.

The BabyCare program is a home-based program that fosters safe and healthy pregnancies and healthy babies by providing free education, care coordination, and support services from registered nurses to high-risk pregnant people and infants up to 24 months of age.
Services Include:

Free child safety seats and booster seats are also available.

CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. Bridges operates an emergency family shelter, known as the Sullivan House, which has approximately 45 beds across 10 separate apartment units. The program provides shelter and case management services to homeless families leading to employment, permanent housing, and achieving self-sufficiency. It is a temporary solution for those with little or no resources. While there, families receive intensive case management - assessing their situation, providing Bridges program resources, or referring to other resources. The families receive whatever immediate needs they may have - food, diapers, personal supplies, basic kitchen items, gift cards and more. The length of stay generally is between 30-90 days, and then Bridges works with the families to move them into permanent solutions
