TOP (Teamwork ~ Options ~ Pathways to Success) is an interest and needs based prevention program designed to connect youth and their families with services that can refocus youth in the right direction by building on their strengths and providing them with a positive support system.

The Care Coordination Program assists older adults (and their families) by helping to coordinate and organize the care they are receiving from various agencies and organizations. Coordinators provide information and referrals; collaborate with other service professionals; and develop person-centered service plans that focus on participants' needs, strengths, and preferences. They also follow up with individuals each month to ensure those plans are working well and adjustment them if any needs have changed.

Care Coordinators answer questions, perform assessments, find and coordinate resources, help apply for benefits, and more. Think of them as the liaison for older adults in the care network focused on addressing the needs of older adults and caregivers.

Provides free access to HIV and Hepatitis C prevention information/education, HIV Pre-Exposure Prophylaxis (PrEP) and non-occupational Post-Exposure Prophylaxis (nPEP) counseling. Service Navigation for linkage to STI testing, PrEP and nPEP navigation, basic needs (i.e., food, housing, etc.), insurance enrollment, etc., available to anyone regardless of HIV or HCV status.

Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to assisting in placement such as an adult home, nursing homes, etc.; companion services designed assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization. Care Transitions is designed to help reduce avoidable readmissions to hospital, by empowering individuals to manage their own health. A Care Transitions Coach will work to transfer skills that will allow an individual to recognize red flags, communicate effectively with their primary care physician, identify medication errors, and keep track of important medical information, with the goal of significantly reducing the likelihood of an unnecessary return to the hospital within 30 days.

Provides services to help prevent and treat Intellectual Disabilities and to enhance the quality of life of individuals who experience these conditions in the Cities of Buena Vista and Lexington and the Counties of Rockbridge and Bath, Virginia by providing individualized treatments, establishing and expanding programs and support systems, and promoting integration into the community. Offers group homes and adult day activities programs to enhance the quality of life for adults with intellectual disabilities by providing comprehensive support services.

Commonwealth Catholic Charities provides a combination of long term or short-term housing subsidies and comprehensive case management services to assist low-income persons living with HIV or AIDS establish or maintain stability in housing, acquire the resources and supports necessary to maintain housing, and access the health and supportive services necessary to thrive in the Greater Richmond region.

Adult Services are provided to adults age 18 and older who have a disability and to their families when appropriate. Adult Services may include the provision of case management, home - based care, transportation, adult day care, nutrition services, placement services, long term care pre-screening for Medicaid-funded services, guardianship monitoring, and other activities to aid the adult.

A family focused program that provides early intervention services for infants and toddlers from birth through two years of age with developmental delays, atypical development, or diagnosed condition. Provides activities for parents to use with their children to encourage developmental growth. Services include: developmental screenings; multidisciplinary assessment; home visits; center-based activities; service coordination, speech and motor therapy. Services are person-centered and community based. Offers services to older children including children's summer camp, respite services, developmental training, day supports, and case management.

The provision of home and community-based services for adults and adults with disabilities to achieve self-sufficiency and to prevent unnecessary institutionalization.
