Provides outpatient-based services designed to evaluate and treat mental health and/or substance abuse disorders in persons age 5 through 18 years of age. Specific services include: Screening, assessment, and treatment planning Counseling/psychotherapy (family, individual, and/or girls' group) Counseling/psychotherapy (individual, group, and family) Crisis intervention (may sometimes be appropriately provided under this core service area or under emergency services) Psychiatric evaluation and treatment Case Management and Service coordination

Assists low-income families with children up to age 6 and pregnant women with access to medical and dental services and act as a link to other human service organizations/providers in the NRV. Offers comprehensive case management in the home where children receive consistent and comprehensive care in a familiar setting. CHIP services, provided by a nurse and a home visitor team, include helping families make and keep appointments and follow physician recommendations, educational home visits, parenting education, helping families set and reach personal and family goals, transportation to medical and dental appointments, medical case management, and referrals to other community-based services to help meet the multifaceted needs of families. All services emphasize prevention and early intervention. This is not the state CHIP insurance, but will help families access Medicaid or FAMIS (state CHIP insurance). Not a substance abuse/ mental health provider but can connect families to those resources. Transports only if no other means available.

Assists adults with brain injuries and their families in developing a plan and obtaining services for maximizing independence and quality of life. Services provided include: information and referral, assessment, long-term case management, independent living skills training, job training/finding/coaching, transportation, respite care, socialization and recreation, mental health and behavioral services, day activity programs, and supervised independent living. Case Management Services for adults provides individuals with serious mental illness the opportunity to collaborate with a caring professional who will provide ongoing support through assessment, monitoring, and advocacy. Individuals will be provided education and linked to services and resources with the goal of meeting that individual's needs and enhancing their quality of life.

Provided to adults and children with serious mental and/or substance use disorders. Case managers provide ongoing comprehensive needs assessment and link individuals to services to help them stay in the community and achieve maximum recovery and quality of life.

Provides community-based case management to child, adolescent, and adult brain injury survivors and their families who reside in the service area. Identifies, plans, coordinates, monitors, and evaluates services and resources to meet an individual's needs. A collaborative process is used to identify survivor centered goals, and marshal (coordinate) resources to meet those goals. Includes advocating for and creating community resources needed to assist survivors in maximizing recovery, optimizing employment or service potential, and reintegrating into their families and communities. Areas which are frequently the focus of case management services include: * Behavior / Self Direction; * Cognitive Skills and Communication; * Education; * Health and Medical Services; * Mobility and Transportation; * Recreation and Socialization; * Self Care and Independent Living; * Vocation and Employment.

Assess the needs of the client and recommend services which will help enable the client to live as independently as possible. The Case Management Department will then coordinate the needs of the client with all available LCAAA and Community Resources. At the time of the initial assessment, a Plan of Care will be formulated, and all clients will be reassessed annually or as their conditions/needs change. Case Management clients will be reassessed bi-annually.

The BabyCare program is a home-based program that fosters safe and healthy pregnancies and healthy babies by providing free education, care coordination, and support services from registered nurses to high-risk pregnant people and infants up to 24 months of age. Services Include: - intensive case management - coordination of care - Pack 'N Plays - education about prenatal care, birthing, infant care, parenting and community resources. Free child safety seats and booster seats are also available.

The DARS Community Rehabilitation Case Management Services (CRCMS) Program assists people with severe physical and sensory disabilities to build a quality of life of their choosing through self-direction, support, and community resources. CRCMS Rehabilitation Specialists provide individualized long-term case management to people across the Commonwealth, as well as outreach and support services to residents of nursing facilities. CRCMS also provides Support Coordination Services to recipients of Virginia's Development Disabilities Medicaid Waiver. Offers instruction in independent living skills including personal finance, cooking and housekeeping, instruction in Braille keyboarding, orientation and mobility training, the use of white canes, and transportation systems. Services provided include teaching techniques that enable individuals with visual impairments to maintain and retain independence.

Mental health case management works to ensure that service systems and community supports are responsive to the changing needs of adults with serious mental illness. This service seeks to assist individuals in gaining access to needed medical, psychiatric, housing, financial, social, educational, vocational, and other supports essential to meeting basic needs. The individual is a partner in assessing their own needs, obtaining services and other supports, monitoring those services, and in preventing and managing crises. Case management utilizes a person-centered approach with a focus on promoting health and recovery.

Bridges operates an emergency family shelter, known as the Sullivan House, which has approximately 45 beds across 10 separate apartment units. The program provides shelter and case management services to homeless families leading to employment, permanent housing, and achieving self-sufficiency. It is a temporary solution for those with little or no resources. While there, families receive intensive case management - assessing their situation, providing Bridges program resources, or referring to other resources. The families receive whatever immediate needs they may have - food, diapers, personal supplies, basic kitchen items, gift cards and more. The length of stay generally is between 30-90 days, and then Bridges works with the families to move them into permanent solutions
