A center and community based program for people with intellectual disabilities that serves ages 12 years through adulthood. Provides supports, as identified in Individual Support Plans, to increase socialization, adaptive skills, communication, and more through the use of community resources (field trips, volunteer opportunities, etc) and center-based activities (arts and crafts, personal hygiene education, horticulture, etc.).

A 34-bed assisted living facility for adults who require assistance with at least two activities of daily living. Takes Alzheimer's patients. Provides respite care.
Adult Day Support for individuals with intellectual disabilities. Services include 10 to 11 activities each month within three programs: self advocacy (education); adapted wellness (fitness); and quality of life (social/recreational).
Provides temporary, substitute care for people with intellectual disabilities in the place of care normally provided by the family or other caregivers. Offered in the consumer's home, the respite provider's home, or in a community respite setting. Also provided on an emergency basis when caregivers are unexpectedly hospitalized or otherwise unable to provide normal care.

Support with activities of daily living, such as personal care, dressing, physical movement or medication management. Experienced 24-hour support team.
Provides the at home caregiver of an eligible patient time off from his/her duties. The program helps to maintain the health and well-being of the patient and the caregiver. This respite program can provide up to 720 hours per year for qualified patients in the Medicaid program. Provides caregivers with relief from care giving duties in an effort to maintain a quality life, independence, and the prevention of premature institutionalization. These duties include meal preparation, shopping assistance, light housekeeping and supervision. There are also Support Group Meetings provided for caregivers. This program helps to maintain the health and well being of the patient and the caregiver.
Provides assessment, planning referral, monitoring and advocacy to individuals with mental retardation, developmental delays and developmental disabilities. Offers respite care. Provides group homes with a supervised home-like environment. Offers limited financial assistance to families for other programs which will enhance the consumers ability to live in the community. Adult Development Day Program - opportunity to learn skills necessary for daily living as well as social interaction. Provides Case Management and transportation for clients and supported employment at The Attic thrift store. Early Intervention - home-based program for families with developmentally delayed infants and toddlers up to age three.

Provides solutions for those who need short-term assisted living care. Flexible program can accommodate most any short-term care need, whether it be for a week, a month, or a little longer. Offers around the clock respite care, daytime only respite care, nighttime only respite care and the flexibility to accommodate most any situation. Respite residents receive the same quality services and amenities as permanent residents. ACCESSIBILITY: Fully accessible to persons with disabilities. Manor House operates its own transportation.
Short-term services are geared towards those who would like to stay in our facility for a temporary period of time. Sometimes, your loved one needs a change of routine, or his or her caretaker needs a break. Whatever the case may be, we welcome residents for any period of time (minimum of four days), whether it be a week to several months. Services include acute, sub-acute and intermediate rehabilitation programs for the orthopedic, cardiovascular and neurological patient. Provides intermediate care in a traditional nursing facility setting, respite care, Alzheimer's care, and hospice care. Offer our patients individualized pharmacy services, vitalstim therapy, in-house rehabilitation, and skilled nursing.

Provide an array of outpatient services for individuals age 18+ with mental illness, substance abuse disorders and intellectual disabilities. Serves adult men & women with the primary diagnosis of Intellectual Disability (diagnosis must be documented as originating prior to age 18). Provide care, assistance and supervision to individuals in their own homes. These services are supplemental to the care provided by a primary caregiver or they may support an individual whose level of independence does not require a primary care provider. Each resident has a plan of care outlining support needs. This plan outlines specific outcomes expressed by the individual or evaluated as an area of need/challenge. The plan is used to help each individual progressively strengthen his/her own individual knowledge, skills and independence.

Attendant Services for People With Disabilities
Provide respite to caregivers of those who are 60 years old and older. JMU students are trained to offer this service and make two hour weekly visits throughout the semester in Harrisonburg and Rockingham County.
Richfield offers short-term, temporary care options in both of our assisted living facilities (The Oaks and Joseph C. Thomas Center) and the Recovery & Care or Rehab Centers. Our Vacation & Recovery Stay Program is ideal for: * Family Members who need to take a break from their everyday responsibilities as caregivers. Our program affords family members the opportunity to vacation, travel, deal with business or personal affairs, or simply take a needed rest from the stress and strain of round-the-clock caregiving. * Patients who have completed a hospital stay or rehabilitation, but feel they would benefit from an added period of professional care before returning home. * Those who have undergone same day surgery or other outpatient procedures and want a day or two of rest before resuming their daily routines. * For assisted living, this program also gives prospective residents and their families the opportunity to evaluate the benefits of assisted living at Richfield before making a permanent move.
