The ACT program comprises multidisciplinary mental health professionals organized as a mobile unit to provide the treatment, rehabilitation, and support services that persons with severe mental illnesses need to live successfully in the community.
Provide goal directed training to enable individuals to achieve and maintain community stability and independence in the most appropriate, least restrictive environment. Each individual will be assessed and have a personalized service plan to best serve their individual needs.

Assesses the consumer's needs for housing, medical care, employment, etc. and seeks to help the individual obtain these resources. This approach is long term and indefinite, with efforts prioritized to those at risk of rehospitalization. Case management services include hospital liaison and discharge planning. Maintains responsibility for the consumer as the person enters the psychiatric hospital and while an inpatient, the case manager works with the family, inpatient staff and community services board programs to insure an effective plan to return to the community.

In-home/community based mental health skill building and support to individuals age 18+ who suffer from severe mental illness. Provides services to adults with serious emotional illness who are in need of support and service in the home to permit or enhance functioning in the community. Also serving those with developmental disabilities and/or Substance Abuse as secondary diagnosis. Initial/ongoing assessment and treatment. Medication management; therapeutic groups; job skills training/obtainment and retention; housing assistance; family support and education; substance abuse services; coordination of healthcare services; community involvemen;, functional skills and appropriate behavior related to the individual's health and safety; activities of daily living, and use of community resources; assistance with medication management; and monitoring health, nutrition, physical condition.
Allows the client to acquire, retain, or improve the self-help, socialization, behaviors and adaptive skills necessary to reside successfully in home and community-based settings in order to prevent an out-of-home placement.

Mental Health Skill-Building Services (MHSS) works collaboratively with adults, age 18 years and older, to build skills that promote self-determination and empowerment. It provides training and education to assist individuals as they work towards stability, independent living, and bettering their daily lives. Individuals eligible for this service may have a dual diagnosis of either mental illness and intellectual disability or mental illness and substance use disorder.

Mental Health Skill-Building: Assists those with mental health diagnoses in meeting all of life's struggles and challenges - from helping the client to access community resources to training the client in the most basic life skills. Provides mental health supports meeting a wide range of needs associated directly with the mental illness itself and also with the accompanying issues that go along with every mental health diagnosis. SA/MH Outpatient Outpatient services consist of individual, group, and family counseling depending on the individuals need. Services will be offered at minimum an hour each week and treatment will cover topics such as coping skills, mental health wellness, anger management, medication assisted treatment (MAT), and other concerns persons with substance abuse and/or mental health issues may face.

Offers mental health supports to adults with serious mental illness. A qualified staff member works one on one with individuals to teach them how to develop coping skills, access community resources, develop a natural support system, and locate safe and affordable housing. Staff members also teach cooking skills, laundry and cleaning skills, nutrition, health and safety, budgeting skills, and community leisure skills. Individuals work toward attaining their goal of independence in the community.
